By completing this payment and upon Synergy Gateway’s approval, you will be granted an account with Synergy Gateway in its Verified software platform for participation in its COVID-19 Vaccination Verification (CVV) program. 

 

Our client(s) have contracted Synergy Gateway to provide access to its Verified software platform for all external contractors/vendors, students and employees. 

Our client(s) have requested that you have access to the CVV program to assist them in verifying the status of your COVID-19 vaccination. 

 

There is a cost to participate in this program, which you will be required to pay upon completing your registration with Synergy Gateway.

IMPORTANT
 

• Please carefully enter the information required below and ensure it is accurate.

• It will take 24-48 hours for you to receive an account activation email.  

• Please look for an email from Synergy Gateway (helpdesk@synergygateway.com) with the subject: "Your Access to Verified" as this will contain your account username and password.   Please check your spam and junk mail folders if you do not see it in your Inbox folder. If you have not received your activation email within 48 hours, kindly go to: https://verified.sgappserver.com/ and click the "Forgot Password/Resend Password" and follow the instructions.
